Latisse Rebate Offers

$50 REBATE: Buy two Latisse boxes, receive a rebate of $50 on the second box.
$100 REBATE: Buy two Latisse boxes and a JUVEDERM® dermal filler treatment, receive a rebate of $100.
 
To claim your mail-in rebate, complete these five easy steps:

1. Read the Latisse REBATE Terms and Conditions (available at purchase).
2. Complete the Claim Form and check the tick box to indicate the REBATE offer being claimed.
3. Include all three items below in your envelope:
• The original Claim Form with the required information filled out and the correct rebate box checked.
• A copy of your Latisse purchase receipt (must show number of boxes and valid purchase date).
• Two Latisse box flaps with UPC codes (photocopies are not accepted).
  
4. Read and complete the “For My Records” section on the reverse form.
5. Mail to: LATISSE REBATE, P.O. Box 224, Station A, Etobicoke, ON M9C 4V3
